Intake and appointments — three simple steps to becoming a client.

Becoming a client happens in three steps. Everything below the first step — intake forms, appointments, secure messaging, payments — lives in the client portal, outside this website.

Please do not share health information in this form. This form is for general questions only. Clinical details should be shared after we’ve scheduled a consultation, through the secure intake portal.

Step 1 · Complete intake forms

A free 20-minute phone call. We’ll talk briefly about what brings you in, and you can ask any questions you have. If we decide to move forward, I’ll send you the intake packet.

Step 2 · Attend intake session

These sessions will take 30-50 minutes where we will discuss client’s history and put together a treatment plan.

The intake link will be sent to you after your consultation.

Step 3 · Attend your first session

Sessions are 50 minutes. You can attend in person or by phone. Between sessions, you’ll use the client portal for messaging, scheduling, and payments.
