
Becoming a client happens in three steps. Everything below the first step — intake forms, appointments, secure messaging, payments — lives in the client portal, outside this website.
Please do not share health information in this form. This form is for general questions only. Clinical details should be shared after we’ve scheduled a consultation, through the secure intake portal.
Step 1 · Request a consultation
A free 20-minute phone call. We’ll talk briefly about what brings you in, and you can ask any questions you have. If we decide to move forward, I’ll send you the intake packet.
Step 2 · Complete intake forms
Intake forms are completed in our secure client portal. They cover basic history, consents, and insurance information. Your intake forms are submitted directly to the secure records system and are never stored on this website.
The intake link will be sent to you after your consultation.
Step 3 · Attend your first session
Sessions are 50 minutes. You can attend in person or by secure video. Between sessions, you’ll use the client portal for messaging, scheduling, and payments.

If you’re already a client, you can schedule, message, or make a payment any time through the client portal.
